The number of people living with Type 2 diabetes has risen exponentially in the last decade, and research has shown that it is no longer effective for institutions to merely “prescribe” what patients should do. What is now needed is consistent coaching tailored to a person’s situation and health motivations to change behaviours.
Remedy Healthcare has devised a coaching program to help those who are living with Type 2 diabetes take control of their health and to live well with diabetes. This program compiles content that has been peer-reviewed by a panel of some of Australia’s top medical specialists, and sits within Australian clinical guidelines.
The chance that both short and long term complications will arise increases the longer you are living with diabetes, so something had to be done to ensure that each person becomes an expert on their own healthcare.
The program is a 6-month phone-based consultation with a trained coach who, much like a swim coach, is with you every step as you head towards your health goals. They review your nutrition habits and, if necessary, switch some of your food choices to healthy, low GI options so that you can feel better and get a better handle on your blood glucose levels. This will also help you to lose weight and up your activity/exercise regime.
Your coach will have current evidence-based information from Diabetes Australia and the Royal College of General Practitioners, and will translate that information to best suit your situation. They will guide you towards recommended tests, check-ups and measurements, will tell you what the results mean and help with medication management.
For anything that you’re unsure of, your coach is there every step of the way to provide knowledge that you may not have access to. No matter how firm a grip you have on your diabetes health management, each consultation will be tailored specifically to you and your needs.
Another great way to take control of your Type 2 diabetes is to look into CBHS’s Chronic Disease Management Programs (CDMPs). These programs are designed to help those with chronic diseases achieve the best quality of life, and have proved to be invaluable to hundreds of our members.
